Please read and confirm the following: I understand that if I am not employed by CYT Fredericksburg, I must supply my own certificate of liability for use of the space. If under 18, I understand a parent or guardian MUST be at the studio at all times. I agree to abide by the following guidelines:
Set-up/Clean-up is the responsibility of the renter. No food or drink is allowed in the main studio. Use of the space is restricted to the lobby/greenroom, kitchen, restrooms and the studio, therefore, no one will be allowed in the office side, the warehouse, or costume area, unless specifically requested in advance. Use of the mats is strictly prohibited, unless granted permission in advance. Studio space MUST be returned to pre-event condition.
-- Tables and chairs cleaned and wiped down.
-- All rooms (tables, chairs, equipment, mats, etc) returned to the same location they were at prior to the event.
-- All leftover food removed and properly disposed of from counter, refrigerators, and freezer.
-- All decorations/paper removed and properly disposed of from building.
-- Restrooms checked/any major problems resolved.
-- Mop up all spills/stains on floors.
-- All counters and sinks wiped clean.
-- Kitchen floor swept.
-- Thermostat reset to 65.
-- ALL Trash cans emptied (lobby, kitchen, restrooms, studio)
-- Trash bags delivered to outside dumpster
-- Any damage must be reported immediately to CYT staff so proper steps could be taken for clean-up and/or repair. I understand that submission of this form is only a request and CYT Staff has right to deny usage.
